The NextGen Practice Solutions (HSI powered by NextGen) Profile: A Comprehensive Overview

NextGen Practice Solutions (HSI powered by NextGen) is a full-service healthcare revenue management company servicing healthcare clients nationwide. HSI’s approach is focused on accounting systems, internal controls, and technology solutions. Our model is achieved through a comprehensive understanding of the revenue cycle including, but not limited to, billing and collection services.

HSI focuses its review on the existing charge/coding capture systems and internal control points in place and assists clients in establishing controls that capture the appropriate revenue and provide a mechanism to bill and collect properly. HSI also reviews and negotiates payer contracts as they relate to billing. The result is optimized collections and identification of lost revenue.

We are one of the most experienced, technology-driven, revenue-improvement companies in the country. HSI strives to consistently improve the efficiency and effectiveness of our clients’ operations. This is done by transitioning from aging technology and outdated processes toward state-of-the-art solutions and best practices. HSI and its partners offer solutions in billing and collection, electronic medical records, point-of-service charge capture, digital imaging and records management, process redesign, and software training and support.

HSI services include accounts receivable management and healthcare consulting for hospitals, independent facilities, device companies, and physician practices. Core services include receivable and reimbursement management and healthcare consulting including billing and collection services, with additional service offerings for medical coding and documentation education, payer credentialing services, and full-service information technology solutions. Through these services, HSI works on behalf of clients nationwide to optimize the revenue cycle through best practice solutions.

HSI currently provides revenue management services to thousands of providers including health systems, hospitals, and physicians in private practice. Our in-house team consists of billing specialists, Certified Medical Coders, Certified Public Accountants, and technologists who know the business of medical billing inside and out.

We currently utilize multiple practice management platforms for our customers allowing each customer the ability to choose the system that fits their needs appropriately. We have developed comprehensive policies and procedures for each system and have internal resources for training and 24/7 software support.

HSI Deliverables
Revenue Cycle Review and Redesign
As a result of our experiences within the healthcare industry, we have the expertise and “know-how” to help our clients review their revenue cycle and assist them in developing a state-of-the-art professional component billing and collection system.

In addition to improving the current state of billing and collections, HSI serves as a business partner to our customers in identifying and implementing revenue enhancement opportunities as discussed in this overview.

Billing and Collection Services
Billing and collection services are at the core of our solution. Designed and built by accounting professionals, our billing and collection processes are based on internal controls, checks and balances, appropriate audit trails, and diligent work. Our solution encompasses the entire revenue cycle from registration to charge capture to claim submission to claims follow-up to payment posting to contract management to financial reporting and analysis.

Our solutions are designed to meet the needs of each customer and are focused on not disrupting their current workflow.

Managed Care Contracting
Given the current healthcare environment, HSI has been instrumental in working with payers on issues regarding claims processing and reimbursement. With robust reporting and access to benchmark data throughout the country, HSI has been successful in optimizing revenue with managed care plans through improved medical coding and increased reimbursement models.

Certified Professional Coders
HSI has a full-time staff of certified coders including Certified Professional Coders (CPC), Certified Professional Coders - Hospital (CPC-H), Professional Medical Coding Curriculum Instructors (PMCC), and Certified Compliance Professionals (CCP). These individuals perform the following services:

  • Comprehensive medical record coding including up-to-date HCPCS, CPT, and ICD codes.
  • Chart Review and Audit with detailed report of findings including revenue enhancements and compliance risk.
  • Coding and Documentation training and instruction based on current Centers for Medicare and Medicaid Services (CMS) guidelines.
  • Annual CPC Preparatory class and exam proctoring.
  • Compliance plan development and review.

Payer Credentialing and Contracting
HSI utilizes the Vistar Technologies VIP Credentialing Software to credential and re-credential providers with government, managed care, and other insurers. The software is completely automated credentialing software that includes provider information storage, expiration notification, imaging, automated form processing, on-line faxing, and ad-hoc reporting.

The software provides the HSI credentialing department the ability to track the credentialing/re-credentialing process for each provider for all payers. Vistar also tracks licensure requirements and expiration dates in order to maintain a comprehensive, up-to-date inventory of each provider. As with all credentialing projects, the correct and timely credentialing for every physician is critical. Without the proper payer credentialing, charges will not be reimbursed. HSI understands the importance of this role and has developed checks and balances to ensure that as soon as a new provider is identified, the payer credentialing process begins.

HSI also utilizes the Council for Affordable Quality Healthcare (CAQH) universal credentialing data source, where appropriate.

Interfaces and Connectivity
HSI maintains a full-time, on-site staff of Information System support personnel. The 10-member team is comprised of data programmers, technical support personnel, system administrators, Help Desk personnel, and Virtual Private Network (VPN) support.
The expertise of this department allows us to connect to and interface with all of our customers in order to utilize technology to enhance our services. We are able to remotely connect to customer information systems and/or develop interfaces to access information more easily and timely. The result is a more comprehensive solution for our customers.

As an added benefit, HSI has the ability to receive and access all financial institution lockbox information in electronic format, including copies of insurance and patient checks, insurance explanation of benefit (EOB) statements, correspondence, and deposit slips with certain banks. Utilizing this technology, HSI is able to access payment/denial information electronically in order to post payments, work denials, and resolve payer and patient issues. This process results in more time spent working unpaid accounts receivable and less time shuffling paper (e.g., EOBs, checks, and so forth).

Communication
It is HSI’s belief that teamwork is the root of all success. As a result, HSI encourages and promotes frequent communications including meetings with our customers and their staff. In addition to regularly scheduled financial review meetings, HSI will coordinate meeting with the various departments related to the auditing and review process. Examples include:

  • Physicians, Clinical, and Administrative Staff
  • Facility/Entity Executives & Management
  • Contracting Department
  • Information Systems
  • Risk Management
  • Registration Department
  • Quality Control
  • Other

HSI maintains a full-time, on-site staff of Information System support personnel. The 10-member team is comprised of data programmers, technical support personnel, system administrators, Help Desk personnel, and VPN support. We rely on this group to work closely with our clients to ensure both a seamless transition and ongoing system support.

PowerFlow Document Management Solution
HSI utilizes the PowerFlow Captured Information Management system to reduce inefficiencies as a result of processing paper. PowerFlow is a feature-rich yet uniquely simple and cost-effective document management software solution designed to eliminate paper and electronic document-related business problems: lost and misplaced documents, limited storage space, and wasted search time causing lost productivity and poor customer service for our customers.

The ability to effectively scan, capture, index, store, archive, and retrieve business documents and communications is critical to the competitive and financial health of our business. Our employees and customers benefit from business communication processes that are digitally enhanced and not “bound” by the countless limitations of the traditional paper file folder.

PowerFlow provides access to the right information by the right people at the right time so that our organization can work smarter and more efficiently and resolve outstanding accounts receivable for our customers more timely and effectively.

Compliance/HIPAA
Compliance
HSI is dedicated to providing services that are compliant with all government regulations and legal requirements. The management team and staff are dedicated to the implementation, operation, and monitoring of our compliance program. HSI began the process in September of 1999, with the development of a compliance plan in accordance with the office of the Inspector General guidelines.

HSI hired the firm of Lindquist Avey/Kroll O’Gara, a national consulting firm specializing in healthcare compliance, as well as the law firm of Nelson Mullins Riley (based in Atlanta, GA) to assist in the development and implementation of the plan.

HSI has an active compliance committee as well as full-time staff dedicated to the operation and monitoring of the plan as a third-party billing and consulting company.
HSI also maintains a toll-free, confidential Compliance Hot Line.

As stated previously, HSI employs both certified hospital coders and certified physician coders as part of our full-time staff. These coders—as well as our billing and collections staff—are available as resources to customers. For example, HSI provides coding in-services to customers on a routine basis. The in-services typically consist of Evaluation and Management (E/M) documentation guidelines, CPT, and ICD-9 coding. In addition, HSI offers a 16-week coding review program that ends with HSI proctoring the CPC exam. These coders also play a vital role in operation and monitoring of our compliance plan.
Any coding issues identified by HSI would be brought to your attention in order to ensure that coding is in compliance with the appropriate guidelines.

HIPAA
HSI faces the same challenges that all healthcare providers are faced with regarding HIPAA-compliance. With the help of outside consultants, HSI has incorporated HIPAA policies and procedures related to transaction code sets, privacy, and security into our existing compliance plan. HSI management believes that the organization has taken the necessary steps and established procedures to provide HIPAA-compliant services including:

  • Global-Certs Secure E-mail Gateway (encrypted HIPAA-compliant email)
  • Facility Access Controls
  • Security Procedures
  • Employee Training
  • Password Management
  • Muzak Speech Privacy System
